Housing Coordinator or SPOC discovers unreported income...next steps
- Housing Coordinator request verification in writing from participant of unreported
income for that specific time period. Additional verification can also be retrieved from
SPOC (FSR, CBMS, DOLE). Do not use the amount of wages in EIV as verification, always
have a second form of verification for wages. - Housing Coordinator sends SPOC the following information:
- Verification of the unreported income
- Family Reporting Form(s) for the unreported income timeframe
- Zero Income Reporting Form(s) if applicable for the unreported income timeframe
- DOH will review and completed the repayment agreement calculation within 10 days
- SPOC will email the Housing Coordinator the calculation worksheet
- Housing Coordinator will schedule a tenant conference within 2 weeks of receiving the calculation worksheet from DOH
- Housing Coordinator will send the signed conference documents and repayment agreement to their SPOC within 10 days of the tenant conference.
SPOC will reach out to Housing Coordinator when repayment agreement is paid in full OR if two consecutive payments have not been received.
No Offer of Repayment Agreement
- Except in the cases of reasonable accommodation, DOH generally will not enter into a repayment agreement with a family if there is already a repayment agreement in place with the family or if the amount owed by the family exceeds $5,000.00.
Two Consecutive Missed Payments
If a participant has missed two consecutive payments, SPOC will reach out to Housing Coordinator and request that a tenant conference for nonpayment is conducted within the next 30 days.
Tenant conference should address the non‐payment issue, discuss if a resolution can be reached, and offer a lower monthly payment if needed. If the participant’s income has changed since the repayment agreement was originally signed, no formal approval for a lower payment is needed and Housing Coordinator may approve lowered amount. To document this, Housing Coordinator may have the participant sign a new Repayment Agreement document with the lower payment amount or simply include information about the lower amount in the conference paperwork. Housing Coordinator sends SPOC completed tenant conference documents.
Request for a lower payment
If a participant wants to request a lower payment outside of a tenant conference, the Housing Coordinator may approve lower payment amount if the participant’s income has decreased. If the Housing Coordinator does not see a decrease in income or is unsure, they send an email to the SPOC for review. If approved by Housing Coordinator, simply send an email to the SPOC
letting them know new monthly payment amount.
Housing Coordinator will not deny a request for a lower payment amount, but instead will send information to SPOC. If denied, DOH will send out a letter to participant, ccing the Housing Coordinator with explanation of the denial.
