Elite Standard Formatting
Standard Formatting and Punctuation
In order to ensure we have good data and it’s easy to search and find what we’re looking for in Elite, all the entries for names, address, and related information must be standardized. To do this, we rely on the format suggested by the United States Postal Service. Everything entered into Elite should be entered in the following format.
You might find it helpful to refer to these lists when entering data into Elite.
Elite Standardization
- Do not use all capital letters. When entering data into Elite, use proper capitalization.
- When entering an individual’s name into Elite, use the Social Security card as a guide. The name needs to match SSA’s records so that we may obtain EIV information.
- Example: Social Security card lists name as “Michael T. Jones”. Make sure you enter “T” as the middle name in Elite.
- Example: Social Security card lists name as “Michael Thomas Jones”. Make sure you enter “Thomas” as the middle name in Elite.
- Example: Social Security card lists name as “Michael Jones”. Do not enter anything in the middle name box.
- Do use the “Suite” box when creating new units. You must enter Apt, Ste etc in the Suite Box prior to the unit number.
- Use “Official USPS Abbreviations” when creating new units (see below). For further clarification, visit the USPS abbreviation page.
- When the address consists of a building number and an apartment number, USPS recommends the following formats:
- Example: Building A Apartment 12 should be Apt A-12 (when there is a building number, you do not need to use the building abbreviation)
- When the address is an apartment that consists of both a letter and a number:
- Example: Apartment A12 should be Apt A12
- Example: Apartment 1B should be Apt 1B
Common USPS Abbreviations
- North, South, East, or West – Only use the first letter. (Example: 125 N Elm St)
- Northwest, Southwest, etc. – NW, SW, etc.
- Do Not Use Punctuation
Secondary Unit Designator (**Do Not Use Punctuation**)
- Apartment – Apt
- Suite – Ste
- Lot – Lot Street
Suffixes (**Do Not Use Punctuation**)
- Avenue – Ave
- Boulevard – Blvd
- Circle – Cir
- County Road – County Road (spell this out)
- Court – Ct
- Drive – Dr
- Highway – Hwy
- Lane – Ln
- Parkway – Pkwy
- Street – St
- Way - Way

Income Source Codes
The following chart will make more sense in later sections of the manual, but here is a list of the correct source codes for each type of income entered into Elite. If you have an income source not listed on this chart, please contact your DOH rep for exactly how to code it.
Please make sure to note there is a difference between SSI and SS/SSDI in the coding.
