In State Transfer Process
DOH has jurisdiction throughout the state of Colorado. DOH’s Housing Choice Voucher applicants/participants have the option to move their voucher to another county within the State. If a DOH Housing Choice Voucher applicant/participant moves within the state of Colorado, DOH will continue to administer the voucher through a partner agency.
DOH’s “other” voucher programs (CoC PSH, SHV and CATV) must be approved by DOH staff prior to the applicant/participant’s request being submitted.
If there are any program violations, a participant conference must be held prior to transfer.
Step One: Applicant/Participant submits Request for In-State Transfer and collects documentation:
- Residential Coordinator receives the Request for In-State Transfer and submits to their SPOC
 - Collect a 30-day notice or Mutual Rescission of Lease
 - Issue a manual voucher for 120 days (not in elite)
 - Process an annual reexamination if one is due within the next 120 days
 
Step Two: Submit paperwork to DOH
- Send the Family Request for In State Transfer, 30-day notice/mutual rescission of lease, manual voucher, and annual reexamination (if needed) to the appropriate SPOC.
 - DOH will then:
- Review paperwork for completeness.
 - Send an email approval of Family Request for In State Transfer to receiving and initial partner agencies.
 - Issue a voucher in Elite with the new increment (receiving agency)
 
 - Initial Agency will maintain the original file for 5 years and send the following documents to the receiving agency:
- Original application
 - All family ID documents (birth certificates, SS card, ID’s etc.)
 - 30 Day notice to vacate or mutual rescission of lease
 - All documents associated with the last effective annual reexamination
 - 50058
 - All income, expense and asset verifications
 - All releases (including the HUD 9886)
 - Any approved reasonable accommodations
 - Any tenant conferences, violation documentation, etc.
 
 
- Receiving agency tracks the voucher clock and is responsible for granting extensions as necessary.
 - Receiving agency will complete the move action.
 - If the applicant/participant does not lease up with the receiving agency within the voucher timeframe, the receiving agency must notify the DOH Contract Manager.
- Initial agency will then complete the End of Participation (EOP) action/process and turns over the voucher to the next person on the initial agency’s waiting list.
