HQS Paper form can be used in place of physical inspection to streamline lease up process

Effective Immediately

In the event an agency inspector is unable to physically inspect a unit due to extenuating circumstances, the CAT-V program allows the HQS Form to be completed in the interim. The paper HQS Form can be submitted with the RFTA to save time during lease up.

Physical HQS inspection must be scheduled upon receipt of the HQS Form, and must be completed within the next 30 days.

DOH will recoup HAP funds from lease up date to present if the unit fails on physical inspection.
